We are seeking a Patient Care Coordinator (PCC) to work closely with our patients and physicians to deliver high-touch, personalized care. This role includes both clinical and clerical responsibilities, such as communicating with patients via phone, email, and text; triaging patients to identify emergent and non-emergent situations; scheduling appointments; obtaining prior authorizations; and submitting prescriptions to pharmacies. The ideal candidate is a Registered Nurse (RN) with at least three years of combined clinical and administrative healthcare experience, who enjoys detail-oriented administrative work and thrives in a fast-paced environment that requires multitasking and timely completion of tasks.

We’re looking for a Clinic Resource Nurse (RN) to work closely with our patients and physicians to provide high-touch, personalized care. The Clinical Resource Nurse (CRN) is a highly skilled Registered Nurse who serves as a clinical expert and trusted advisor for nursing staff and interdisciplinary team members. This role enhances patient care delivery by providing real-time clinical guidance, mentoring, and support. The CRN functions as a key resource to strengthen clinical decision-making and foster continuous improvement within the team. The ideal candidate has at least 3–5 years of clinical nursing experience, preferably in outpatient or ambulatory care.
